SDLC Phases and their I/P & O/P

Gurus,

I have listed the documents involved in all the phases of SDLC in a project below. Kindly let me know if I have missed anything in this list:

Planning Phase:
Input: Application Goals
Output: Project plan & Schedule, QA Plan, Configuration Plan

Requirement Phase:
Input: HLD (Project Plan)
Output: Requirement Document, Updated HLD Document, and Requirement Traceability Document.

Design Phase:
Input : Requirements Document
Output : Design Document, Updated HLD & Schedule, Updated Req Traceability Document

Development Phase:
Input : Design Document
Output: Software, Test Plan, Implementation Plan

Test Phase:
Input : Software, Test Plan, Implementation Plan
Output: Integrated Software, Acceptance Plan, Production Initiation plan

Installation & Acceptance Phase:
Input : Integrated Software, Acceptance Plan, Production Initiation plan
Output: Acceptance Test, Archived Project plan, Archieved Software

    Please note the planning is not a phase but a process which will run concurrent to all the phases of a project lifecylce. You never do planning only once, right! Every phase of the SDLC requires planning.
